What is MASLD?<\/h2>\n\n\n\n<p>MASLD is a chronic metabolic liver condition defined by hepatic fat accumulation in the absence of significant alcohol consumption and in the presence of at least one feature of metabolic dysfunction. This term replaces the previous definition of NAFLD (non-alcoholic fatty liver disease), in accordance with the international consensus (AASLD, EASL, ALEH, APASL, 2023).<\/p>\n\n\n\n<figure class=\"wp-block-image article-content-thumbnail-wrap\"><img decoding=\"async\" src=\"https:\/\/vitamed.production.am\/wp-content\/uploads\/2026\/05\/MASLD-MASH.png.jpg\" alt=\"METABOLIC DYSFUNCTION-ASSOCIATED STEATOTIC LIVER DISEASE (MASLD)\"\/><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\">2. Etiology and risk factors<\/h2>\n\n\n\n<p>MASLD develops in the context of metabolic dysregulation, including:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Abdominal obesity<\/li>\n\n\n\n<li>Insulin resistance<\/li>\n\n\n\n<li>Type 2 diabetes mellitus<\/li>\n\n\n\n<li>Dyslipidemia (\u2191triglycerides, LDL; \u2193HDL)<\/li>\n\n\n\n<li>Hypertension<\/li>\n\n\n\n<li>Genetic predisposition<\/li>\n\n\n\n<li>Sedentary lifestyle and poor diet<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">3. Classification<\/h2>\n\n\n\n<ol class=\"wp-block-list\">\n<li>MASLD without inflammation (simple steatosis)<\/li>\n\n\n\n<li>MASH \u2013 with steatohepatitis, ballooning, and fibrosis<\/li>\n\n\n\n<li>Hepatic fibrosis (F0\u2013F4)<\/li>\n\n\n\n<li>Cirrhosis secondary to MASH<\/li>\n\n\n\n<li>Hepatocellular carcinoma (HCC) \u2013 may occur even without cirrhosis<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading\">4. Clinical features<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Asymptomatic in 70\u201380% of cases<\/li>\n\n\n\n<li>May present with fatigue, right upper quadrant discomfort<\/li>\n\n\n\n<li>Often detected via elevated ALT and AST<\/li>\n\n\n\n<li>Transaminases may remain normal, especially in MASH<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">5. Diagnostics<\/h2>\n\n\n\n<p>Initial workup:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Abdominal ultrasound (limited with BMI >30)<\/li>\n\n\n\n<li>Liver enzymes: ALT, AST, GGT, ALP, bilirubin<\/li>\n\n\n\n<li>HOMA-IR, lipid profile, glucose<\/li>\n<\/ul>\n\n\n\n<p>Non-invasive fibrosis assessment:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><p>Elastography (FibroScan)<\/p><\/li>\n\n\n\n<li><p>FibroTest, FIB-4, NAFLD fibrosis score<\/p><\/li>\n\n\n\n<li><p>MR elastography (if uncertainty remains)<\/p><\/li>\n\n\n\n<li><p>Liver biopsy \u2013 gold standard for confirming MASH and fibrosis stage<\/p><\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">6. Treatment (AASLD\/EASL recommendations)<\/h2>\n\n\n\n<p>1.&nbsp;Lifestyle modification (first-line):<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>\u22657\u201310% weight reduction<\/li>\n\n\n\n<li>Mediterranean, DASH, or low-glycemic index diet<\/li>\n\n\n\n<li>\u2265150 minutes\/week of aerobic physical activity<\/li>\n<\/ul>\n\n\n\n<p>2.&nbsp;Pharmacotherapy (for MASH or fibrosis \u2265F2).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Pioglitazone (especially in T2DM)<\/li>\n\n\n\n<li>GLP-1 receptor agonists: semaglutide, liraglutide<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">7. Complications<\/h2>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Progressive liver fibrosis<\/li>\n\n\n\n<li>Cirrhosis and portal hypertension<\/li>\n\n\n\n<li>Hepatocellular carcinoma<\/li>\n\n\n\n<li>Increased cardiovascular risk<\/li>\n\n\n\n<li>MASLD-associated mortality exceeds liver-specific complications<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">8.
